Income, Expense and Debt Forms

When filling out the income worksheet, use your paycheck stub if possible to fill in the fields. Your gross income (income before taxes) from all sources will determine whether or not you are above or below the median income for the state. This will help us determine whether or not you belong in a Chapter 7 or a Chapter 13.

Expense Sheet (click to download form)

When filling out the expense worksheet, use your current monthly living expenses. Some items you may not buy every month. For those types of items, add up your annual expenditures for that item and divide by twelve. That will give you a monthly expense for that item. For example, you might buy clothes for the kids right before school starts, at Christmas and on their birthday. Add up what you spend each time then divide by twelve. This is an average monthly living expense.

When filling out your debt worksheet, estimate the amount of the debt to the best of your ability. Don't worry if it is not exactly the amount owed. If you hire us, we will pull your credit report which will have all of the creditors that report to the credit reporting agency with the amounts owed.
